Our Medical Records Department understands the importance of confidential and safe record keeping. We fully comply with the Health Insurance Portability and Accountability Act (HIPAA). If you need to request a copy of your medical records, our staff will be happy to assist you.

Birth Certificates/Social Security Card:
If you have questions regarding the social security card for your baby, please call Marie Aschliman at 801-538-6366, Utah Vital Records or contact the Social Security Administration Customer Service Center at 1-800-772-1213. You may also visit the SSA website at www.ssa.gov. In Utah, it usually takes three weeks from the date of birth to receive the card.
